Ontario’s Personal Health Information Protection Act, 2004 (“PHIPA”) requires health information custodians to maintain privacy practices and provide a public statement explaining how personal health information is handled.

Online Booking and Intake Forms

TLC Medicine uses Jane Software Inc. (“Jane”) for online booking, clinic administration, and patient intake forms. When you book through Jane, you may be asked to provide contact information, appointment details, health history, treatment consents, insurance information, and other information needed to arrange and provide care.

Your intake questionnaire is completed through Jane’s patient portal. Jane provides the technology platform that supports booking, intake forms, clinical records, billing, and related clinic functions. TLC Medicine controls the patient information within its Jane account and manages who within the clinic may access it. Jane’s intake forms are designed to collect and securely store information such as health history, consents, and billing details.

How We Use Information

We may use personal information and personal health information to:

  • Deliver a guide, resource, or information you requested.
  • Respond to inquiries and appointment requests.
  • Schedule, administer, document, and provide clinical care.
  • Send appointment-related messages, forms, reminders, receipts, or other service communications.
  • Send newsletters, clinic updates, educational resources, and promotional emails where you have provided consent.

Email Marketing

If you provide your email address to receive a guide, newsletter, educational resource, or other marketing communication, TLC Medicine may send you email communications with your consent.

Our email communications may be managed using Kit (formerly ConvertKit) or another email service provider. Your name and email address may be processed by that provider to send the resource you requested and, where you have consented, occasional marketing or educational emails from TLC Medicine.

You may unsubscribe from marketing emails at any time by selecting the unsubscribe link in an email or by contacting us at admin@tlcmedicine.ca. Unsubscribing from marketing emails will not stop essential communications about existing appointments, care, billing, or service requests.

Canadian anti-spam rules require consent, sender identification and contact information, and a working unsubscribe mechanism for commercial electronic messages.

Contact Forms

If you contact us through our website, we may collect your name, email address, phone number, and the content of your message so that we can respond to your inquiry.

Our general contact form is not intended for urgent matters, medical emergencies, or detailed health information. If you are experiencing a medical emergency, call 911 or seek immediate emergency medical assistance.

Cookies, Analytics, and Advertising

TLC Medicine uses cookies and similar technologies to operate and improve our website, understand visitor activity, measure advertising effectiveness, and help visitors find relevant information about our services.

Google Analytics: We use Google Analytics to understand website traffic and visitor interactions, including pages viewed, approximate geographic area, browser and device information, referral source, and engagement with our website. TLC Medicine does not intentionally send names, email addresses, contact-form content, booking details, symptoms, diagnoses, or other personal health information to Google Analytics.

Meta Pixel: We use Meta Pixel to measure the effectiveness of advertising on Facebook and Instagram, understand certain actions taken on our website, and help show relevant advertisements to people who visit our website. Meta may receive technical information such as browser, device, IP address, pages viewed, and website actions. This information may be associated with a person’s Meta account if they are logged in to a Meta service.

Google’s privacy information explains that its services use data for analytics and measurement, while Google Analytics policies require disclosure of tracking and data-processing practices.

Access to patient information is limited to people who require it to provide care, administer clinic services, or meet legal and professional obligations. We retain information only for as long as needed for the purposes described in this policy and in accordance with applicable legal, professional, and recordkeeping requirements.

No method of electronic transmission or storage is completely secure. While we take reasonable steps to protect information, absolute security cannot be guaranteed.
